Facilities and Care
There are currently 4 wet rooms, situated on each floor, each individual service user will have a detailed risk assessment of needs, and specialist equipment will be supplied on identified need, there is a large lounge and a large summer room. To the rear of the garden there is a purpose - built communal area with seating and safety flooring throughout. Large landscaped garden to the front of the Building just off the car park, with seating area and disabled access.
We are not a “specialist” provider but we do have a wealth of experience and knowledge within our staff group in meeting the physical, psychological and social needs of people who have mental health issues.
All potential service users of Plas Isaf are needs assessed prior to offering a placement to ensure that we have the knowledge and skill set within our team to meet the needs of the prospective service user.
There is currently a health Emergency with increasing numbers of frail elderly people being sent to hospitals which are struggling to cope, hospitals are currently struggling to meet the needs and provide person centered care, common causes of hospital admissions are UTI, confusions and respiratory conditions, many of these are preventable if treatment is readily available and ongoing support immediately initiated.
Here at Plas Isaf we recognise dementia is unique to individuals, mood and behaviour may be profoundly affected but individuality of that person remains the same, we offer a range of support including-

- Therapeutic interventions to tackle symptoms
- Community connections
General Health Care and Treatment working alongside local GP surgeries. District Nurses, Dietitians and or including agencies such as salt teams, physiotherapists - Mental Health Care where required with CMHTs
- Regular care reviews with all care teams.
